Steps
Go to the Form Library
Open the Form Library section of your platform.Create a New Template
Click the New Template button on the right-hand side.Enter Template Details
Template Name: Example — Site Survey
Description: Add context for whoever will receive or fill out the form (e.g., customers or staff).
Choose which fields are mandatory (e.g., First Name, Last Name, Email, Number).
For customer forms → Include all four.
For internal forms → Only include what’s necessary.
Click “Create Template”
Your base template will now appear with the mandatory fields you set.Add More Questions
Click the Add Question button.
Choose the question type:
Text Answer: lets users type freely.
Multiple Choice: lets you define selectable options (e.g., Grass, Hard Standing).
Add as many questions as you need.
Save the Form
Click Save once all fields and questions are added.Share Your Form

Choose to send a link or email the form directly to the recipient.
